Family Plan


The Family Plan service agreement includes repair and/or replacement of external equipment that is not functioning due to normal wear and tear, one annual preventative service*, and access to Cochlear’s troubleshooting services.

The Family Service Agreement plan is available to families, as defined in the service agreement, with two or more, but not more than six, individuals who have a Nucleus® cochlear implant. Dependent family members means the following: a family member who is dependent upon the parent or guardian listed for financial support and for whom a deduction will be claimed on the parent(s)’s or guardian(s)’s Federal Income Tax return for the year this form is completed.

*Theft/loss/damage coverage is limited to one claim per system component per 3-year period. Claims will be honored only if a current Service Agreement is in place. Coverage includes theft, loss or damage due to misuse, negligence, accident or disaster. All claims must be substantiated with a police report or notarized letter explaining theft, loss or damage in case a police report is unobtainable. Claim form must be completed by recipient’s audiologist.
